For decades, lung cancer has been viewed as a disease almost exclusively linked to cigarette smoking. While smoking remains the leading cause of the disease, doctors are increasingly diagnosing lung cancer in people who have never smoked a single cigarette. This trend is becoming more apparent in India, where a significant proportion of lung cancer patients, particularly women; have no history of tobacco use.
The growing number of cases among non-smokers has prompted researchers to look beyond cigarettes and examine other risk factors, including air pollution, indoor smoke exposure, occupational hazards, genetic mutations and lifestyle-related influences. Experts stress that while non-smokers generally have a lower risk than smokers, the absence of a smoking history does not eliminate the possibility of developing lung cancer.
Lung Cancer Is No Longer Just A Smoker’s Disease
Lung cancer remains one of the most common cancers worldwide and a leading cause of cancer-related deaths. In India, oncologists are increasingly reporting cases in individuals who have never smoked, challenging the long-held perception that tobacco is the only significant cause of the disease.
One reason for delayed diagnosis in non-smokers is the widespread misconception that lung cancer affects only smokers. As a result, persistent respiratory symptoms are often attributed to allergies, infections, asthma, or pollution, allowing the disease to progress before it is detected.
Doctors emphasise that anyone experiencing persistent respiratory symptoms should seek medical evaluation, regardless of their smoking history.
Dr. Shyam Krishnan, Lead Consultant, Clinical & Interventional Pulmonology, CMRI – CK Birla Hospitals, Kolkata said, “While smoking remains the leading risk factor for lung cancer, an increasing number of cases are now being diagnosed in people who have never smoked. This trend highlights the need to look beyond tobacco as the sole cause. Prolonged exposure to air pollution, second-hand smoke, indoor pollutants from biomass fuel used for cooking, occupational exposure to carcinogens such as asbestos and silica, and genetic mutations are all recognised contributors.”
Dr Krishnan also highlighted that in India, worsening air quality in urban centres is an emerging concern, with long-term exposure to fine particulate matter (PM2.5) believed to increase the risk of lung disease, including lung cancer.
Air Pollution: A Growing Concern
One of the biggest contributors to lung cancer risk among non-smokers in India is air pollution. Many Indian cities consistently report high levels of fine particulate matter (PM2.5), which can penetrate deep into the lungs.
Long-term exposure to polluted air causes chronic inflammation and oxidative stress, both of which can damage lung cells over time. Several international health agencies classify outdoor air pollution as a carcinogen because of its association with lung cancer.
People living in heavily polluted urban areas may experience years of continuous exposure, increasing their cumulative risk even if they have never smoked.
Indoor Air Pollution Often Goes Unnoticed
While outdoor pollution receives considerable attention, indoor air pollution remains an under-recognised risk factor.
In many households, particularly in rural and semi-urban areas, cooking with biomass fuels such as firewood, charcoal, crop residue, or cow dung generates dense smoke containing fine particulate matter and harmful chemicals. We are regularly detecting instances of the condition in smokers as well as in those who have never used tobacco. This trend suggests that apart from tobacco use, several different factors are probably playing a role in contributing to the growing number of cases of lung cancer.”
Dr Swami also emphasised that these potential risk factors consist of long-term exposure to air pollution, living close to people who smoke, cooking using biomass fuel like cow dung that smoke produces, exposure to chemical hazards at work, certain places having high levels of radon gas, and changes in the DNA of genes. In India, the use of domestic fuels and inhalation of city smog, for a long time, have been identified as the possible risk factors that could bring up lung cancer in those who had never smoked.
The Hidden Danger Of Second-hand Smoke
Not smoking does not necessarily mean avoiding tobacco smoke. People living or working with smokers may inhale second-hand smoke for years. Tobacco smoke contains thousands of chemicals, including dozens of substances known to cause cancer.
Studies have shown that prolonged exposure to second-hand smoke increases lung cancer risk, particularly among spouses and family members of smokers. Creating smoke-free homes and workplaces remains an important public health measure.
Occupational Exposure Can Increase Risk
Certain occupations expose workers to carcinogenic substances that can damage the lungs over time. Workers in construction, mining, manufacturing, shipbuilding, and industries involving asbestos, silica dust, diesel exhaust, arsenic, chromium, or nickel may face a higher risk of lung cancer if appropriate safety measures are not followed.

Researchers have identified several genetic mutations that occur more frequently in lung cancers affecting non-smokers, particularly mutations involving the epidermal growth factor receptor (EGFR). These genetic alterations are seen relatively often among Asian populations, including Indian patients, and have transformed treatment by enabling the use of targeted therapies.
A family history of lung cancer may also increase the likelihood of developing the disease, although genetics alone rarely explain all cases.

Dr Prasad also noted that one of the biggest challenges is delayed diagnosis, as both patients and clinicians often overlook persistent symptoms such as chronic cough, unexplained weight loss, chest pain, or breathlessness in non-smokers. As a result, many cases are detected at advanced stages when treatment options become more complex.
Increasing awareness that lung cancer can affect anyone is essential. Individuals with persistent respiratory symptoms should seek timely medical evaluation regardless of their smoking history. Advances in molecular diagnostics and targeted therapies have significantly improved treatment outcomes, making early detection more important than ever. With public awareness, cleaner environments, and access to timely diagnosis can help reduce the growing burden of lung cancer among non-smokers in India.
Women Are Disproportionately Affected
A notable proportion of non-smoking lung cancer patients are women. Researchers believe this may result from a combination of environmental exposures, biological factors, and genetic susceptibility.
Women may experience greater exposure to indoor air pollution from cooking, particularly in households using biomass fuels or lacking adequate ventilation. Hormonal influences and specific genetic mutations are also being investigated as possible contributors to differences in disease patterns between men and women.
These findings highlight the need to recognise lung cancer as a health concern that extends beyond tobacco use.
Early Symptoms Are Often Overlooked
